What does a navigator do?
- Guide patients through the care system
- Help with understanding your diagnosis and treatment options
- Connect patients with support services at VHC Health or in the community
- Communicate with your healthcare providers
- Survivorship care planning
- Facilitate support groups and workshops
- Patient advocacy
- Aid in addressing financial concerns
What Does a Navigator Do?
Your patient navigator can help you and your family:
- Understand your diagnosis and treatment options
- Find support services at VHC Health or in the community
- Communicate with your healthcare providers
- Feel emotionally supported through up to three sessions of counseling
