Diabetes Self-Management Program
VHC Health's Diabetes Management Program is recognized by the American Diabetes Association (ADA) for Quality Self-Management Education.
Participants in our programs learn:
- What to eat – comprehensive nutritional guidance on carbohydrate counting, label reading, meal planning, grocery shopping and fitting in favorite foods.
- How to monitor – instructions on using a blood glucose meter or continuous glucose monitor, blood sugar targets and pattern management skills.
- What to expect – education on avoiding or delaying long-term complications, managing medications, avoiding low blood glucose and available support services.
Diabetes Management Assessment Form
Type 2 Diabetes
Individual appointments and/or group classes covering subjects such as healthy eating, weight loss, avoiding complications, and blood glucose monitoring.
Type 1 Diabetes
Individual sessions with a Certified Diabetes Care and Education Specialist (CDE) and monthly online support group for peer help. Topics include carbohydrate counting, insulin titration, and insulin pump training.
Gestational Diabetes
Individual appointments to help ensure a healthy pregnancy through balanced nutrition and controlling blood glucose levels.
You’ll need a referral from your doctor to participate in this program.
